Primary Function:
The Director of Patient Care coordinates the activities of the clinical staff to assure the organization programs and services are consistent with state and federal standards.
The Director of Patient Care provides clinical leadership, guidance, and mentorship to the patient care staff.
Oversees program development and participates in organization planning and oversees patient care expenses.
Ensures the highest level of HCL standard of care.
II.
Qualifications:
a.
Current license to practice as Registered Nurse in the state of South Carolina b.
Experience in hospice and/or home health c.
BSN and Certified in Hospice and Palliative Care preferred d.
Leadership, Supervisory and Management experience required e.
Commitment to hospice philosophy, compassionate end of life care f.
Interpersonal skills to motivate and engage staff to be active in quality improvement activities g.
Analytical, problem-solving skills h.
Direct and concise communicator i.
Well versed in analytics and excellent presentation skills j.
Clinical thinker with solid nursing judgement III.
Physical Demands:
Work requires the physical demands of frequent sitting, written and verbal communication; occasional bending/squatting, reaching above shoulder level, crouching/kneeling, opening and closing file drawers, lifting/carrying and fine manipulation when working with computer and telephone.
Essential sensory requirements are visual acuity, touch and hearing.

V.
Job Specific Duties:
a.
Responsible for access to care including the assurance that admission to hospice services is available 24/7/365 b.
Oversees patient referral process, ensuring all documentation is complete and patient has access to care within 24 hours of hospice referral except when otherwise specified by the patient or designee.
c.
Continually reviews staffing needs of patient care team to assure needs of patients are fulfilled d.
Identifies needs for changes in processes and assists leadership team with performance improvement plans for both efficiency and quality improvement.
e.
Ensures patient care delivery meets all state and federal requirements.
f.
Works collaboratively with Director of Quality and Compliance to assure adherence to CMS and third-party standards of practice.
g.
Serves as leader with all state and federal surveys to assure compliance with regulatory requirements.
h.
Oversees scheduling of clinical staff.
i.
Participates in strategic planning and program development.
j.
Plans and executes staff education for clinical staff when indicated.
k.
Coordinates annual competency training in collaboration with Director of Quality and Compliance.
l.
Lead role in establishing and maintaining updated policies and procedures related to patient care, including annual review.
m.
Responsible for interviewing and selection of qualified patient care staff; ensuring individual orientation based on needs of the new hire with on- boarding as per agency's policy.
n.
Serves as leader in IDG, ensuring thorough review and update of each patient's plan of care o.
Conducts monthly staff meetings with RNs; LPNs and CNAs communicating changes and opens discussion for opportunities for improved services.
p.
Follows through with patient and family concerns to ensure high quality care and customer service is rendered.
q.
Assumes responsibility for agency administration in absence of Executive Director.
r.
Participates in AOC coverage as assigned.
s.
Performs hospice admission assessment when all other resources are maximized.
t.
Carries patient caseload during periods of significant need.

Completes other duties as assigned.

Hospice, Palliative and Bereavement Coordinators as well as patient care RNs, LPNs, CNAs, Social Workers, Spiritual Counselor and contract employees.
