Obtain history of present illness, past medical history, current health status, family health history, psychosocial history, and occupational health history
Develop and execute care plans including assessment, diagnosis, planning, implementation and evaluation
Perform daily head-to-toe geriatric assessment including vital signs and document findings
Perform finger sticks and insulin administration daily
Administer PO, IM, SQ medications and vaccinations
Manage wound care, wound vac, decubiti & PICC line care
Communicate information effectively to primary physician
Provided patient education and rehabilitation support functions to patient/family.
Estimated Salary: $20 to $28 per hour based on qualifications.
